Graham Brown, MPH, Senior Vice President, NextGen® Advisors
Graham Brown is a principal and senior vice president with NextGen® Advisors focused on transforming care with provider organizations. His practice centers on accountable and value-based care strategy, population health management programs, and technology solutions for providers enabling new models of care delivery across the United States.
Mr. Brown is a former senior vice president and national practice leader for population health and clinical integration with GE Healthcare Partners (previously The Camden Group) where he led multidisciplinary client teams in strategy creation, program development, implementation, operations, and performance optimization engagements. He is an experienced leader in organizational development, managed care contracting, and change management initiatives.
Mr. Brown has over 25 years’ experience supporting provider groups, health and hospital systems, integrated delivery networks, and managed care payers to assess, design, contract, and implement systems and structures for population health management. He has worked nationally across the United States and Canada.
Laura Larson-Huffaker, Chief Executive Officer, Horizon Health and Wellness
Laura Larson-Huffaker is Chief Executive Officer of Horizon Health and Wellness. She has over 20 years of experience in non-profit health care leadership. She earned her Master’s degree in Counseling from Arizona State University, and is a Licensed Professional Counselor. Laura has served on multiple local non-profit boards, as well as various community task forces geared toward addressing health care and child welfare needs in the state of Arizona. She was selected by the Virginia Piper Trust Foundation for its 2005 Fellowship Program, during which she attended the University of Pennsylvania’s Wharton School of Business and earned a Certificate of Professional Development with an emphasis on Leadership.
Carmen Heredia, Chief Executive Officer, Valle del Sol
As Chief Executive Officer for Valle del Sol, Carmen Heredia leads a multistate nonprofit organization that provides integrated behavioral health and primary care health to underserved populations. Valle del Sol was founded in 1970 to provide advocacy and leadership training to Latino communities in Phoenix. The organization has expanded into comprehensive federally qualified health centers with eight locations in Maricopa County and nine locations in New Mexico. Having served the community for sixteen years at Valle del Sol in several executive positions, Carmen’s focus has been promoting quality service delivery and organizational excellence.
Carmen is a double-Sundevil, having received both her undergraduate and graduate degrees from Arizona State University. She is Licensed Master Social Worker in Arizona and focused on planning and administration in her degree program. Carmen maintains a CPHQ – professional certificate in health care quality. Carmen also has a certificate from the ASU Nonprofit Executives program.
She is currently serving as the Chairwoman of the Community Board of Dignity Health St. Joseph’s Hospital and Medical Center and Westgate hospitals. Carmen also serves on the boards of the Center for the Future of Arizona, the Arizona Council of Human Service Providers, the Arizona Alliance of Community Health Centers, National Council of Behavioral Health, and was recently invited to serve on ASU President’s Community Council, the Arizona Coyotes Latino Advisory Council, and the Advisory council of the Phoenix Community Development and Investment Corporation. She is an alum the National Council’s Addressing Healthcare Disparities Leadership Program, and Valle del Sol’s Hispanic Leadership Institute. Carmen is a member of Greater Phoenix Leadership.
A leader in her community and organization, Carmen strives for equality and inclusion of underserved populations especially regarding access to quality healthcare and education. As a Mexican immigrant, she passionately volunteers in Latino civic engagement, various political campaigns, and voting efforts with her family. Carmen lives in Mesa, Arizona, with her husband and three children.
Eleanor Castillo Sumi, VP, Research & Program Development, Uplift Family Services
Dr. Castillo Sumi is a licensed psychologist and Board-Certified Behavior Analyst with a wide depth and breadth of experiences in the delivery of mental health services to youth and families. In the area of school mental health, her experience includes an emphasis in training and consulting services with school district personnel on School-Wide Positive Behavior Interventions and Supports (SW-PBIS) and Multi-Tiered Systems of Supports (MTSS). Her practice in mental health, behavioral issues and strategies has led her to work with children with autism spectrum disorder, youth in the juvenile justice system, and to manage a personal business venture.
As a past Best Practices Specialist for the State of Hawaii Child and Adolescent Mental Health Department, Castillo Sumi is well-versed in the most current Evidence-Based Practices. Dr. Castillo Sumi had an earlier incarnation at EMQFF as Corporate Compliance Officer and Outcomes and Quality Assurance Director . Dr. Castillo Sumi has had her work published in several publications (e.g., American Journal of Orthopsychiatry, the National Association of Dual Diagnosis (NADD) Bulletin, etc.). She obtained her Ph.D. in Clinical Psychology from Pacific Graduate School of Psychology at Palo Alto University where she was awarded Dissertation of the Year among her graduating class.
Dawn Cottrell, Chief Clinical Officer, Horizon Health and Wellness
Dawn Cottrell is the Chief Clinical Officer at Horizon Health and Wellness. In this role, she oversees the integrated care services (behavioral health and primary care) for all 7 of the agency’s locations, including 5 FQHC sites. Dawn earned a master’s degree in Social Work from Arizona State University and is licensed by the Arizona Board of Behavioral Health Services. With over 20 years of experience in the healthcare industry, Dawn has an extensive understanding and expertise in best practices, fundamentals in program development in rural communities, and integrated care.
