Making The Health Care Social Service Link: The Optum MyWellnessPath Case Study

In any discussion about finding sustainable overlaps between the social determinants of health (SDH) addressed by social services and health care, there is the financial challenge faced by payers is developing programs that address specific subpopulations that can be defined by various SDH, and treated as a "whole person."
