These are the contract documents for Arizona's Medicaid managed care programs. The state of Arizona operates two primary managed care programs to serve Medicaid enrollees.

Acute Care Program: Nine health plans deliver acute care services to individuals who do not require long-term services and supports (LTSS). Contracts for six of the nine health plans, signed January 28, 2013, are available for download:
