Nascentia Health To Use Biofourmis Care Platform For Aging-In-Place Programs 

Nascentia Health, a home health care agency in New York, is set to use care management and insurance services from Biofourmis to help consumers receive the care they need at home. It will use the Biofourmis Care connected platform to launch and grow care-at-home programs, with the goal of decreasing adverse outcomes and unnecessary hospital readmissions and accelerating transitions to care at home.;
