Northwell Health Partners With NYC & Others To Improve Health Services For Young Adults Transitioning Out Of Foster Care
A new pilot program in New York is designed to improve health care services for young adults ages 20 to 26 that are transitioning from the foster care system to adulthood. The program is run by health system operator Northwell Health, New York City’s Administration for Children’s Services, the Center for Fair Futures, and three foster agencies serving Queens.
The new Youth Healthcare Link program will coordinate life and skills coaching offered by Fair Futures with Northwell’s consumer navigation services. The program is expected to enroll 50 to 100 participants and connect them with health care navigators . . .
