Community First Choice State Plan Option: Technical Guide

In January 2017, the Centers for Medicare & Medicaid Services published this technical guide for the Community First Choice (CFC) state plan option as a development and implementation plan for the program. The technical guide includes information on eligibility; service models and service budgets; required and optional services and provider qualifications; home- and community-based settings; assessment of need and service plan; and service delivery systems.
