New York State’s Application for the CMS Integrating Care for Dual Eligible Individuals Initiative

January 28, 2011 New York State applied to participate in the federal Centers for Medicare and Medicaid Services (CMS) Integrating Care for Dual Eligible Individuals Initiative. In the application, the Department of Health described its intention to explore ways to manage funding to deliver integrated Medicare and Medicaid services.
