This document was released by the Centers for Medicare and Medicaid Services (CMS) on January 30, 2019. It entails an electronic, web-based survey that will allow states to self-report their progress in implementing electronic visit verification for personal care services and home health care services, as required by section 1903(1) of the Social Security Act.
