This document was released by the Centers for Medicare and Medicaid Services (CMS) on January 30, 2019. It entails an electronic, web-based survey that will allow states to self-report their progress in implementing electronic visit verification for personal care services and home health care services, as required by section 1903(1) of the Social Security Act. CMS will use the survey data to assess states’ compliance with section 1903(l) of the Act, and to levy federal medical assistance percentages reductions where necessary, as required by 1903(l) of the Act.
