Minnesota Health Care Homes: Annual Report On Implementation: Report To The Minnesota Legislature 2012- 2013

The report was released on January 31, 2014, by the Minnesota Department of Health (MDH) and the Minnesota Department of Human Services (DHS). MDH, DHS, and the University of Minnesota evaluated the state's implementation and outcomes of the Medicaid Health Care Home initiative from July 2010 through December 2012.
