Evaluation Of The State Of Minnesota’s Health Care Home Initiative; Evaluation Report For Years 2010-2012

The report was released on January 31, 2014, by the Minnesota Department of Health (MDH) and the Minnesota Department of Human Services (DHS). MDH, DHS, and the University of Minnesota evaluated the state's implementation and outcomes of the Medicaid Health Care Home initiative from July 2010 through December 2012. The evaluation found that 43% of all primary care clinics in Minnesota are now certified as health care homes. These clinics serve nearly 3.3 million individuals.
