Medicare Fee-for-Service 2015 Improper Payments Report, Supplementary Appendix

The report was released in April 2016 by the Centers for Medicare & Medicaid Services (CMS). It presents CMS calculations of the Medicare Fee-for-Service (FFS) improper payment rate as determined through the Comprehensive Error Rate Testing (CERT) program. CERT evaluates a statistically valid random sample of claims submitted during the year to determine if they were paid properly under Medicare coverage, coding, and billing rules.
