June 13, 2011 The Florida Agency for Health Care Administration (AHCA) notified Mapleway Community, Inc., that its Medicaid provider agreement would be terminated as of July 13, 2011. Mapleway Community operates three assisted living facilities that serve clients of the Florida Agency for Persons with Disabilities and the Florida Department of Children and Families. The three facilities have been the subject of numerous complaints that residents were subjected to physical, emotional, and sexual abuse.
