Oregon Patient-Centered Primary Care Home Model Implementation Guide
August 15, 2011 Oregon is moving toward a primary care medical home model and has implemented procedures for recognizing health care practices as patient- centered primary care homes (PCPCHs). The PCPCHs are a key element of the stateÂ’s health care reform transformation to coordinated care organizations (CCO), and PCPCHs will be a required element of CCO provider network.
