Minnesota Accountable Health Model Request For Proposals For The Accountable Communities for Health Grant Program
On December 4, 2014, Minnesota's Commissioner of Health and Commissioner of Human Services selected 12 Accountable Communities for Health to each receive a $370,000 grant, out of a total grant amount of $4.4 million.
