Minnesota DHS Olmstead Plan Transition to Community Initiative Memo

This document outlines new services to be offered by the Minnesota Department of Human Services to reduce the state's reliance on institutional care. The services are included in the state’s first Olmstead plan as activities to be conducted to ensure that state residents with disabilities are able to live and receive needed services in the least restrictive setting possible. In 2011, about 10% of the 5.3 million Minnesota residents had a disability.
