October 11, 2011 The initiative created by the Centers for Medicare & Medicaid Services (CMS), Center for Medicare and Medicaid Innovation seeks proposals from commercial and state payer organizations interested in participating in a four-year demonstration focused on enhanced care management in primary care practices. Under the initiative, primary care practices in selected markets will receive a monthly per-patient per-month payment to cover the costs of providing enhanced care coordination and access.
