Implementation Of Oregon’s Patient Centered Primary Care Home Program Program: Exemplary Practice & Program Findings
In September 2016, the Oregon Health Authority (OHA) released this report about the statewide impact of the state's Patient-Centered Primary Care Home (PCPCH) program launched in 2012. This report evaluates the implementation of the PCPCH program and aims to understand the organizational conditions and process improvement activities of "exemplary" PCPCH clinics. The researchers also estimated the impact of the PCPHC program on the utilization and expenditure patterns of clinics' consumers; and assesses the general consistency and usefulness of PCPHC scoring in evaluating performance.
