Evaluation Of The Medicaid Health Home Option For Beneficiaries With Chronic Conditions: Annual Report Year Three

In July 2015, the federal Department of Health and Human Services released the third year annual report regarding the evaluation of the Medicaid Health Home option for beneficiaries with chronic conditions. This report examines providers’ experience with the program, focusing on implementation challenges, provision of required services, and use of health information technology. Findings are drawn from review of background materials and each State Plan Amendment, as well as qualitative data collected during site visits and annual follow-up telephone interviews with state program staff, health home providers, consumer advocates, and other key stakeholders.
