This presentation on Health Homes is from the Minnesota Department of Human Services. Section 2703 of the ACA defines health home services as comprehensive and timely high-quality services provided by a designated provider or a team of providers and specifically include: care management; care coordination; health promotion; transitional care; patient and family support; referral to community and social support services; and improved exchange of health information.
