On July 21, 2015, Humana Inc. and AMC Health launched a new partnership to provide a telehealth post-hospital discharge transition of care program to reduce the risk of readmission among Humana’s Medicare Advantage members. Through the partnership, AMC Health will support Humana At Home, Humana’s national division that supports about 900,000 people with chronic health conditions. AMC Health will provide a 30-day, post-discharge Interactive Voice Response (IVR) program to 1,000 Humana Medicare Advantage members in 35 states who have been recently discharged from the hospital and are at risk for readmission.
