On December 11, 2019, Humana announced agreements with care management companies Monogram Health and Somatus to provide new kidney disease care coordination services to eligible Humana commercial and Medicare Advantage members. The personalized, coordinated services are available through Monogram Health in Louisiana and Mississippi, and through Somatus in Georgia and Virginia.

Services for eligible Humana members in these states include coordination with a member’s primary care physician and nephrologist; in-home health and medication assessments; education about treatment options such as home dialysis; and emotional support through counseling.
