In July 2013, the Center for Integrated Health Solutions released a report discussing Medicaid health home models in five states currently including serious and persistent mental health condition or substance use disorder as eligible chronic conditions under their health home benefit: Missouri, New York, Ohio, Oregon, and Rhode Island.
