Unite Us Launches Patent-Pending Integration Between EHRs & Its Social Care Coordination Platform

Unite Us recently announced the launch of a patent-pending integration between electronic health records (EHRs) and its Social Care Coordination Platform. The effort focuses on integrating clinical and social information to deliver whole person care, as well as address the social determinants of health.

The integration is timely, as increasing evidence shows that up to 80% of health is determined by the social determinants of health, or the conditions under which individuals are born, grow, live, work, and age.
