Charter Health Care Group acquired two hospice service provider organizations in Colorado and Utah. It acquired Vitality Home Healthcare, which does business as Phoenix Home Care & Hospice in Denver, Colorado; and acquired Heartwood Home Health & Hospice, in Salt Lake City, Utah. Financial terms of the transactions were not disclosed.
