Family Health West To Close The Oaks Assisted Living

Family Health West (FHW) in Fruita, Colorado announced the closure of The Oaks Assisted Living, which is owned and operated by The Lower Valley Hospital Association dba FHW, effective August 1, 2021. FHW said “ongoing operations in The Oaks Assisted Living have become imprudent. The aging facility has gone beyond its useful life, and the facility’s limitations have begun to hamper the ability to continue to deliver high-value, high-quality care.”
