83% Of Safety Net Medical Home Pilot Sites Achieved PCMH Recognition Within Four Years
Of the 65 safety net health care sites participating in an initiative to develop patient-centered medical homes (PCMH), 54 (83%) of the sites achieved external recognition that they were functioning as PCMHs within four years. The remaining nine sites had implemented some of the key design features of the PCMH Model of Care.
