Michigan To Launch All-Payer Patient-Centered Medical Home Initiative In 2017

During 2017, Michigan plans to launch an all-payer patient-centered medical home (PCMH) initiative as part of its state innovation model (SIM) grant. Physician practices participating in the PCMH Initiative will receive payment through Michigan’s Medicaid managed care organizations, the fee-for-service (FFS) Medicare program, and participating commercial payers. The Michigan Department of Health and Human Services (MDHHS) is in the process of designing a payment model approach for implementation beyond 2017 with participating practices and payers.
