Minnesota Medicaid To Launch Assessments For Statewide HCBS Transition Plan For Home & Community-Based Settings

On November 17, 2014, the Minnesota Department of Human Services (DHS) released the Medicaid statewide transition plan governing home- and community-based services (HCBS) to ensure that HCBS waiver services are provided in qualifying settings by December 2018. The settings must comply with federal rules for community settings, person-centered planning, integration, and choice. The transition plan applies to Minnesota’s 1915(c) waivers: the Brain Injury waiver, the Community Alternative Care waiver, the Community Alternatives for Disabled Individuals waiver, the Developmental Disabilities waiver, and the Elderly waiver. DHS will accept comments on the plan through December 17, 2014.
