New Hampshire To Pilot Two Programs Aimed At Helping Dual Eligibles Avoid Institutional Care

In late July 2023, the New Hampshire Department of Health and Human Services (DHHS), Division of Long-Term Services and Supports (DLTSS) released two procurements to help Medicare-Medicaid dual eligibles avoid entering institutional care. One procurement, request for proposals (RFP-2024-DLTSS-07-COUNT) will award grant funds to one of New Hampshire’s 10 counties to test a model proposed by the county.
