Pennsylvania Medicaid Chronic Care Medical Homes Reduced Annual Per Person Costs By $4,000
Pennsylvania Medicaid beneficiaries with co-morbid medical and behavioral health disorders who were enrolled in a primary care medical home through the Chronic Care Initiative (CCI) had fewer hospitalizations, which reduced the average per person cost of care by up to $4,100 annually. The CCI was a large state-wide patient-centered medical home (PCMH) initiative in Pennsylvania in place from 2008 to 2011. The CCI primary care practices were structured as a “medical home” that provided team-based primary care that also included education, behavioral health support, and chronic care coordination all in one place.
