On May 26, 2022, SCAN Group in Long Beach, California acquired home health organization The Residentialist Group (TRG) to form Homebase Medical. The financial terms of the acquisition were not disclosed. Homebase Medical will serve SCAN Health Plan members, as well as members of other health plans. Currently, Homebase Medical serves individuals in California, Pennsylvania, and Tennessee. The goal is to improve the health and independence of older adults.

Homebase Medical Group provides palliative care, chronic disease management, care transition management, and in-depth personal health assessments to people in their homes. The services can be provided via house calls . . .
