Strive Health & Evolent Care Partners Create Kidney Care Collaboration

On April 12, 2022, Strive Health and Evolent Care Partners (ECP) announced the launch of a kidney care initiative for primary care professionals (PCPs) in ECP's Shared Savings Program accountable care organization (ACO) in the Michigan and North Carolina markets. The initiative will help the PCPs manage care for Medicare beneficiaries attributed to the ACO who have advanced stages of chronic kidney disease (CKD) and end stage kidney disease (ESKD).
