Population Health & Care Management: The Key To Whole-Person Care & Value-Based Care Delivery Models
The trend toward “whole-person care” meaning a fully integrated model of care including physical health, behavioral health, and social determinants of health, is growing. A number of states have created managed care models for children in foster care, two states have added services to address homelessness to their Medicaid state plans, and many more are in the process of creating waivers to allow for that type of flexibility.
