On October 15, 2020, the State of Hawai'i released this proposed evaluation design for the QUEST Integration section 1115 waiver demonstration. The State of Hawai‘i, Department of Human Services (DHS), Med-QUEST Division (MQD) is Hawaii’s Medicaid agency. MQD first implemented QUEST on August 1, 1994. QUEST was a statewide Section 1115 Demonstration project that initially provided medical, dental, and behavioral health services through a competitive managed care delivery system.
