In 2020, about 73% of commercial health insurance markets (280 of 384) metropolitan statistical areas (MSAs) in the U.S. were highly concentrated. A highly concentrated commercial health insurance market is defined as one in which the Herfindahl-Hirschman Index (HHI) is greater than 2500. In an analysis of 2020 membership data, 91% of MSA-level markets had at least one insurer with commercial market share of 30% or more. In 46% of MSA-level markets, a single insurer had commercial market share of 50% or more. The HHI is a measure of market concentration.
