On July 1, 2019, Legacy Healthcare took over operations of the 16 remaining South Dakota nursing facilities in receivership. Chicago-based Legacy Healthcare is an affiliate operating partner of Cascade Capital Group. Cascade is a private health care real estate investment and management company, which oversees more than 135 facilities in 23 states.

The 16 facilities, now branded under the name Avantara, have more than 1,000 beds. After the merging of two facilities, the remaining 15 facilities are located in Groton, Ipswich, Milbank, Redfield, and Watertown. Fourteen of these facilities are nursing homes, and one is an assisted living . . .
