New Jersey Awards $3.4 Million To Transition People With I/DD From Nursing Homes To Community Settings
On September 4, 2024, the New Jersey Department of Human Services (DHS) awarded over $3.4 million in grants to help 57 people with intellectual/developmental disabilities (I/DD) transition from institutional settings to community-based housing and services. The grants were awarded to seven provider organizations that serve people with I/DD. The grants are through the federal Money Follows the Person program. This work enhances ongoing efforts by DHS and the Office of the Long-Term Care Ombudsman to move individuals with I/DD out of nursing homes and developmental centers into community-based settings.
