Whole Person Care In Action: The MyCHN Case Study

For many provider organizations, the ability to serve consumers with the most complex needs remains a strategically strong market differentiator. The key to doing this well is finding the best model to meet that market demand. Often that takes understanding the options for operating high-performing, whole person services.

During this one-hour live presentation, the MyCHN leadership team of Yvette Poindexter, MD, Chief Medical and Operations Officer; Penny Pabst, Chief Administrative Officer; and Nydia Garcia, Quality Director, will discuss MyCHN’s integrated model, which offers hybrid care through telehealth visits, remote patient monitoring with connected medical devices, and digital AI mental health support.

Upon enrolling in this model, consumers sign a comprehensive consent for medical, dental, behavioral health, and lab services. Before their first visit, behavioral health consumers receive vital signs, and if necessary, they receive navigation to same- or next-day primary care. Primary care consumers are screened for depression, anxiety, tobacco, alcohol, and drug use and referred to behavioral health services as appropriate. Clinicians are trained to deliver Medication-Assisted Treatment (MAT) and Office-Based Opioid Treatment (OBOT).

  • Review health care outcomes data for the behavioral health integrated patient population compared to overall health center data.

  • Yvette Poindexter, M.D., F.A.C.O.G., Chief Medical & Operations Officer, MyCHN
    Dr. Yvette Poindexter pursued her undergraduate studies at Spelman College, graduating with honors with a Bachelor of Science in biology. She then earned her medical degree from the University of Texas Medical Branch. Dr. Poindexter’s commitment to continuous improvement and innovation in operations management compelled her to pursue a Master of Business in healthcare management. Following her residency and Chief Resident tenure in obstetrics and gynecology at Baylor College of Medicine, Dr. Poindexter became a board-certified Obstetrician-Gynecologist. She joined MyCHN as the Chief Medical Officer in 2010, overseeing the day-to-day operations, ensuring the efficiency and effectiveness of the company’s processes and systems. Under her leadership spanning over 15 years, MyCHN has implemented innovative programs that have improved patient care, outcomes, and experiences, ensuring the center provides the highest standard of care.
  • Penny Pabst, M.ED., OHCC, Chief Administrative Officer, MyCHN
    Ms. Pabst has 27 years of experience working in federally qualified health centers. She oversees quality, risk management, compliance, credentialing, grants development, medical records, and social determinants of health (SDOH) duties. She also oversees the patient satisfaction survey process, private foundation, and the state and federal contracts to ensure compliance with all contract components.  
  • Nydia Garcia, MPH, CPHQ, Six Sigma Certified, Quality Director, MyCHN
    Ms. Garcia currently serves as Quality Director at MyCHN, a multi-site Federally Qualified Health Center (FQHC) and Certified Community Behavioral Health Clinic (CCBHC), where she leads cross-functional teams in advancing care quality through innovation and strategic alignment. She is Chair of the Brazoria County Suicide Prevention and Mental Wellness Coalition and serves on the Behavioral Health Advisory Council committee for Texas Health and Human Services Commission (HHSC). Her certifications include Six Sigma, Certified Professional in Healthcare Quality (CPHQ), and Certified Medical Interpreter. Nydia holds a B.A. from Rice University and a Master of Public Health (MPH) from the University of Texas Health Science Center at Houston.
