Georgia Department of Community Health seeks a contractor to provide the Department with Medical Assistance Long Term Care Transition Coordination for Money Follows The Person service. The contractor will transition Medicaid eligible individuals to receive support for appropriate long-term services in a community setting of their choice. The Transition Coordinators will transition older adults, persons with physical disabilities, and acquired brain injuries residing in an institution and meeting the required level of care into a qualified residence using Money Follows the Person demonstration and supplemental services and HCBS waiver services . . .
