Vermont's Green Mountain Care Board seeks a contractor to provide an evaluation of payment and delivery system pilot projects based on three payment models: bundled payments, hospital/physician budgets, and population-based global payments. In addition to assessing actual performance related to delivery system improvements, the evaluation must assess patient experience, provider experience, caregiver experience, access to care, quality of care, and reduction in the growth of health care expenditures compared to initial projections.
