Illumination Foundation—A Case Study On Integrated Social Support

Health care payers, providers, and regulators are beginning to embrace the inclusion of social supports and human services into treatment plans for people who live with chronic conditions.  We have learned that inclusion of social determinants of health (SDOH) through various interventions could reduce health care expenditures. Many value-based arrangements and plans for targeting underserved populations are focused on SDOH programs, most effectively, programs related to improving homelessness.

The outcomes of programs aimed at reducing homelessness have clearly demonstrated the effectiveness of these interventions in reducing health care expenditures. Other SDOH programs are being funded through the advent of numerous 1115 waivers and Home and Community-Based Waiver Service (HCBS) programs.

At our OPEN MINDS Circle Elite Executive Roundtable Webinar on February 24th, we will be talking with Paul Leon, chief executive officer at Illumination Foundation in Orange County, California, who will share with us how his organization has successfully provided targeted, interdisciplinary services aimed at breaking the cycle of homelessness. Mr. Leon will discuss their programs, describe their 3 Spokes Model, and answer questions from our audience about how helping people with housing, helps to improve health outcomes for homeless adults with serious mental illness and substance use disorders.

  • Paul Leon, Chief Executive Officer, Illumination Foundation
    CEO & Founder of the Illumination Foundation (IF), Paul Leon was instrumental in the conception and development of the Comprehensive Health Assessment Team for the homeless (CHAT-H) and the Mobile Unit Teams (MUT), bringing targeted services to the underserved at the individual, family, community, and systems levels of practice while a County of Orange Health Care Agency Public Health Nurse and pioneered IF’s Recuperative Care Program. Paul consulted on innovative funding methods for recuperative care with the Health Resources and Services Administration (HRSA) across the country and in Puerto Rico. Paul has gained recognition for IF with several awards, including a place on the 10 AARP Well-Being Champions and in 2019, the AARP Purpose Prize Award. In addition to serving on several advisory committees for organizations, Paul mentors and conducts classes for students at Cal-State Long Beach, Cal-State Fullerton, Biola University, Concordia University, USC School of Social Work, and UCI’s School of Medicine. He also consults with CHOC’s Residency Program and the Kaiser Family Resident Program and served on the Board of Directors for the National Health Care for the Homeless Council (NHCHC). Read more. 
  • Richard Louis, III, Vice President, OPEN MINDS
    Richard Louis, III has extensive experience as a behavioral health care administrator, business development specialist, and innovator of new service lines for behavioral health care organizations and currently serves the Vice President -  Western Region at OPEN MINDS. Previously, Mr. Louis was the Director of Development – Behavior & Addiction Medicine at Southern California Healthcare Systems Inc./Prospect Medical. There, he was involved in many consulting projects focused on health care integration, developing new service lines, and health plan contract development. Most recently, he pioneered the development of a series of innovative and profitable integrated behavioral health treatment and population health management solutions that target high cost and complex behavioral health populations. He brings to OPEN MINDS expertise in population health management program development, strategic planning and payer mix diversification, and provider market assessment and growth positioning.
  • Sharon Hicks, MBA, MSW, Senior Associate, OPEN MINDS
    Sharon Hicks, MBA, MSW, has more than 20 years of experience in the health and human service field. She has extensive expertise and a wide range of experience in health plan management, clinical operations management, and technology. A recognized thought leader among her peers, Ms. Hicks is a regular keynote speaker at industry conferences and association meetings, as well as an author of hundreds of articles and resources for professionals in both clinical and executive roles. Prior to joining OPEN MINDS, Ms. Hicks spent two decades in a number of executive positions within the University of Pittsburgh Medical Center (UPMC) system. She served as the Chief Operating Officer for Community Care Behavioral HealthUPMC’s managed behavioral health organization where she was responsible for all aspects of the organization’s operations, including fiscal, information systems, the claims processing department, and the design of clinical systems. She also managed the day-to-day operations of human resources, facilities, purchasing, and security.
