New York's Nassau Health Care Corporation seeks a contractor to provide advice and support with the conversion of traditional hospital-based resident continuity clinics in internal medicine and pediatrics into NCQA 2011 standards level 3 patient centered medical homes. The contractor will have experience in working with academic health centers whose mission includes care of the underserved and underinsured, and who have successfully facilitated transformation of resident dependent teaching clinics.
