Oregon's Multnomah County Department of Human Services, Mental Health and Addiction Services Division awarded a contract to Cascadia Behavioral Healthcare to facilitate continued reduction of the wait list for civilly committed patients awaiting state hospital placement by providing care coordination services to acute care hospital Emergency Departments that facilitate safe diversions from hospitalization. The goals are moving persons from acute or long term care services to community based services; reflecting evidence-based case management practices; and assisting clients who are not already funded through the Mental Health Organization capitation.
