Services will include transporting offenders and families to appointments and meetings; initiating and maintaining weekly contact with all stakeholders involved in the youth's release plan who form a transition support team; physical site visitation during and post-release; and provision of the written monthly reports and verbal communication submitted to MCF-Red Wing program manager outlining successes and challenges with each individual release plan . . .
