Minnesota Department of Health, Office of Rural Health and Primary Care seeks a contractor to provide administrative services for the survey of Minnesota's 79 Critical Access Hospitals (CAH) using the "Minnesota Medicare Rural Hospital Flexibility Program 2005 Administrator Survey". The contractor will summarize survey results, compare 2009 results with 2005 results and submit a report with findings and recommendations. The survey is designed to identify strengths and weaknesses in Minnesota Flex Program activities targeted at designating and supporting CAHs, and identifying current and anticipated rural hospital issues and needs.
