U.S. Department of Health and Human Services, Centers for Medicare and Medicaid Services, Center for Medicare and Medicaid Innovation announces the availability of funding for implementation of the Transforming Maternal Health (TMaH) Model Grant. The purpose of this grant is to test whether targeted technical assistance, coupled with payment and delivery system reforms, can drive a whole-person care-delivery approach to pregnancy, childbirth, and postpartum care while reducing Medicaid and CHIP program expenditures.
