Pennsylvania's Delaware County Department of Human Services seeks a contractor to provide an Extended Acute Care Program. The program will be developed to address Medical Assistance eligible HealthChoices residents of the County who are 18 years of age and older and have an Axis 1 mental health diagnosis. The program will serve both males and females. Many individuals may have a co-occurring diagnosis.
