Better Performance For Existing Whole Person Care—Back By Popular Demand
1:00 – 2:00 pm ET
Over half of specialty health care organizations are delivering or starting to deliver whole person, integrated care. But many executive teams are discovering that operating a successful program that delivers concrete results requires more than simply adding primary care or behavioral health to their service mix. To better manage complex consumers or maximize value-based contracts, organizations need a performance optimization strategy.
During this one-hour recorded presentation, OPEN MINDS Senior Associate Christy Dye and Executive Vice President, Karen Carloni will share how health and human service executives can elevate their clinical and financial performance in whole person, integrated service delivery models. They will present the OPEN MINDS signature program positioning assessment tool while highlighting some of the common roadblocks on the path to a high-performing model, as well as share strategies to optimize performance.
Executive attendees of this session will learn:
- How to conduct a thorough assessment of existing whole person, integrated care programs.
- How to identify methods to enhance clinical workflows and interdisciplinary team communication within the whole person care framework.
- How to identify strategies for financial sustainability and growth within whole person care models.
- How to formulate an action plan based on assessment findings and strategic goals, and how to establish key performance indicators (KPIs) to monitor and measure ongoing success and improvements.
This roundtable was originally presented on April 25, 2024.
