Designing A Measurable, Outcome-Driven Value-Based Care Model: The Community Health Network (MyCHN) Case Study
1:00 – 2:00 pm ET
As executive teams shift from fee-for-service (FFS) to value-based care (VBC), one question consistently emerges: how do you define, measure, and pay for value?
Too often, organizations default to tracking utilization or operational performance metrics. But these measures alone do not capture whether care is improving consumer health outcomes, strengthening treatment adherence, or supporting long-term continuity of care. Without an outcomes-driven framework, many VBC models fall short of delivering meaningful impact.
In this one-hour live presentation, Mark Young, Chief Executive, and Linh Tran, Chief Financial Officer of Community Health Network (MyCHN), will share how their organization redesigned its value-based care strategy to prioritize measurable consumer outcomes over checklist-driven performance metrics.
What This Case Study Explores
This session presents a practical case study examining how MyCHN shifted its VBC approach to focus on outcomes that matter:
- How participation in multiple value-based care arrangements with commercial payers, Medicare Advantage plans, and accountable care organizations (ACOs) revealed an overemphasis on utilization control and documentation metrics
- How operational strategies can strengthen continuity of care across the full treatment journey, including integrated care coordination, improved specialty access through structured provider partnerships, and enhanced pharmacy compliance
- How leadership decisions and targeted strategies can drive measurable improvements in consumer engagement and long-term treatment continuity
What Executive Attendees Will Learn
Executive attendees will gain practical insights, including:
- Recognizing the VBC operational gaps that prioritize utilization control and reporting metrics rather than consumer access and measurable outcomes
- Understanding strategies for building outcome-driven care delivery models that leverage integrated care coordination, specialty access management, and pharmacy compliance oversight
- Applying practical leadership principles for designing sustainable VBC, including how to align resources, partnerships, and operational workflows that measure true impact
