How Your Organization Can Lead The Charge In Whole Person Care To Create A Better Community

The Whole Person Care (WPC) system is a holistic approach to healthcare that looks at vulnerable individuals through a “whole person” lens to address complex needs within their full spectrum. Clinical professionals have pioneered this system to be able to measure a patient’s social determinants of health for low-income communities, including poor nutrition, lack of safe and stable housing, incarceration, and unemployment.

As defined, WPC coordinates health, behavioral health, and social service needs in a patient-centered manner. There is a continuum of modalities in which organizations are implementing WPC. In this session, we’ll discuss how WPC has transformed to help break down barriers as well as what impacts the levels of integrated care and WPC have on patient outcomes. We will also discuss the systemic barriers organizations may face in advancing WPC and how to overcome these. A set of tools will be provided to assist your organization in the pursuit to address the whole needs of each person served.

  • Discuss the benefits of WPC on patient outcomes and how to measure this in your organization
  • Compare the definition of WPC to what’s actually being performed in real clinical settings
  • Learn where your organization currently is in the WPC continuum and what tools, resources, etc. to use to continue to move into a more holistic mindset

Katie Morrow, BSW, MPA, VP of Compliance, Streamline Healthcare Solutions

Katie Morrow is a Licensed Bachelor’s Social Worker with seven years’ experience in the clinical field. In her clinical experience she was a Case Manager for adults with mental illness and developmental disabilities. After receiving a Masters Degree in Public Administration, she transitioned to also doing quality improvement tasks as a Performance Improvement Clinician, which included coordination of The Joint Commission and State audit reviews, data analysis of the electronic health record data, and staff training for her agency on the use of Streamline products.

She began working directly for Streamline in August of 2011. With Streamline, Katie has been the project manager on several implementations as well as providing training and support to Streamline’s customers.
